Annual Nutritional assessments

Is it a new regulation that we complete a full nutritional assessment anually?  I assess each resident quarterly in my progress notes, ie.. appetite, weight/changes, meds, labs, skin etc.. but it seems a consultant during a mock survey said it is a regulation for annual nutritional assessement.  What form do you all use if you do do one?

    I was told the same and with a new, measured height. I hope someone uses a simple form. My narrative is thorough and I recalculate needs only on a change of conndition.

    The only prescribed form in the regulations is the RAI. There are no other specific forms required. An annual assessment can be in narrative form.

      Yes the MDS and COS are the only yearly requirement not a whole new Nutitional assessment form as per my recent surveryors expertice. My usual narritive charting which is comprehensive is ok, but I started to write Annual , Quarterly or Change in the margin where I also write Nutition by the date.
